Overview
TLR4 is a pattern recognition receptor that initiates innate immune responses by detecting bacterial lipopolysaccharide (LPS) and endogenous damage-associated molecular patterns (DAMPs). In the brain, TLR4 is expressed on microglia and participates in neuroinflammation. It is implicated in the pathogenesis of virtually all neurodegenerative diseases through its role in chronic neuroinflammation.
Expression Pattern
TLR4 shows cell-type specificity:
-
Microglia: Highest expression, constitutive
-
Astrocytes: Lower expression, inducible
-
Neurons: Very low, sometimes inducible
-
Endothelial cells: Blood-brain barrier expression
Structure
TLR4 is a ~90 kDa type I transmembrane protein:
-
Leucine-rich repeat (LRR) extracellular domain
-
Toll/IL-1 receptor (TIR) intracellular domain
-
MD-2 co-receptor for LPS binding
-
Forms homodimers
Normal Function
TLR4 functions:
-
Pathogen Recognition: LPS detection (gram-negative bacteria)
-
DAMP Sensing: HMGB1, heat shock proteins
-
Innate Immune Activation: NF-κB, MAPK signaling
-
Neuroinflammation Initiation: Microglial activation
Role in Disease

|---|---|---|
| Alzheimer’s Disease | Aβ recognition | TLR4 binds Aβ; protective and harmful roles |
| Parkinson’s Disease | Neuroinflammation | LPS-induced PD models |
| ALS | Motor neuron inflammation | TLR4 activation in ALS |
| Stroke | Ischemic injury | TLR4 worsens stroke outcome |
